Answering The DoC Questions

The following provides guidance on completing each of the questions

Duty of Care and other Sampling

Quarantine Bay Number.:  Enter the number of the Quarantine Bay into which the load being sampled was placed.  If quarentine bays are not in use, as for example at Willesden rail depot, enter N/A in this field.

Date Load Deposited:  The sample should be taken form the load as soon as possible after it has been deposited.  Enter the date when the sample was actually taken in this field.  This should be the same date as the Sample Date but may differ if the load could not be safely sampled on the day it was deposited. Please note that this is a free text field but the date should be entered as dd/mm/yy.

Company Depositing Load:  Enter the name of the company bring the waste to site and who deposited the material in the quarantine bay

Vehicle Registration Number:  Enter the registration number of the vehicle from which the load was deposited

Waste Ticket Number:  Enter the number from the ticket created by the Company recording system.  This is the ticket produced by the company accepting the waste.

WIF Number:  Enter the WIF number approving the job from which the load came.

Load Address:  The address from which the waste was collected must be entered here.  This should be taken from the Waste Transfer Note accompanying the waste brought to site.

Waste Description:  The load deposited in the quarantine bay should be described in this field.  The description should include a general description of the material in terms of colour and texture and include a description of any visible constituents.  An example might be 'Clean dark grey dry clay with occasional rock boulders up to 40cm across'.  As much information as possible should be provided in this field to aid later interpretation of analytical results.  This is likely to be more expansive than the sample description as the latter will relate solely to what is in the sample container.
