Creating a Chain of Custody

If you are sending data to the lab

1)Click Save to ensure all changes have been recorded

2)Select the Home  tab

3)Select COC icon

4)New  - name the COC using the automatically generated one

5)Check the box Show only with contract (SPM) and select STL-TH in the Lab window

6)Select a COC template (accept the default)

7)Add number of containers (unfiltered – there will be no filtered containers), highlight the rows to which the number applies and click apply

8)If different samples have different container number repeat step 6 for each number

9)Save
 

10)For Planned Samples,

i.Select Add Scheduled Samples (SPM) icon on the top menu bar

ii.Highlight the samples you want to add to the COC – note if you want to add all the samples, and easier way to do this is to:

1.Select Filter

2.Enable

3.Select All

iii.Add.
 

11)For Additional Samples

i.Select Add Unscheduled

ii.From the left hand side of the screen, select the required samples

iii.From the right hand side of the screen select the required analysis

iv.Assign Analysis

v.Finish
 

12)SAVE
 

13)Export to Excel – SAVE – this is the COC

14)In EXCEL ensure you enable Macros, otherwise you will see an empty CoC

a.Save a copy of the CoC as a pdf and print a copy to go in the box with the samples

b.Where there are multiple pages to the CoC make sure you select all pages by clicking on the tab for the first page at the bottom of the screen and then Ctrl click on the last page.  The tabs will turn to a lighter shade to show they are selected

c.email the pdf file to the lab and cc as follows:

i.To SamplesCoventry.UKEnviro@alsglobal.com and meera.dass@alsglobal.com  Cc to Annette.symonds@cemex.com , Karen.magee@cemex.com sue.cullum@ext.cemex.com

ii.PLEASE ENSURE that the words 'CEMEX Site AND [Site Name]' appear in the email header row.

 
NOTE:  Where scheduled samples could not be taken, the sample type should have been set to NST.  Any samples with a sample type of NST will NOT appear on the Chain of Custody as the samples do not exist.  The absence of the NST samples from the CoC is correct.
