NMHA Form

The Near Miss Hazard Alert form is provided to allow any notifiable near misses or hazards through the EDGE system.  The form should be used differently on manned and unmanned sites as follows:

 Manned Sites – Complete the NMHA Form in EDGE but you MUST also complete the paper form provided on the site and hand it to the site manager or foreman.  This will be addressed through the operational site systems once completed in this way.  The electronic form in EDGE will alert National Reserves Department to the reported near miss or hazard so that the issue can be followed up and feed back provided.

 Unmanned Sites – On those sites that are unmanned and/or non operational there is only a requirement to the complete the NMHA Form in EDGE.  The issue will be reported through this process by the National Reserves Department team, action taken and feedback provided.

This process of NMHA reporting is a vital part of the CEMEX safety system and all technicians are strongly encouraged to report any unsafe actions or conditions so that safety can be improved.

The form should be completed as follows:

1.Click on the NMHA Form tab

2.The boxes are all greyed out

3.In order to record a NMHA it is necessary to select a location.  Please click on a location in the Location Pane that is closest to, or related to the nature of the notification

4.Click on New on the left hand side of the third ribbon down

5.You will be prompted to enter a Task Code.  Please select a task that contains the location selected

6.Please then complete each question

a.In the General section, select you name

b.In the Near Miss Hazard Alert Card section complete each of the questions, providing as much information as is necessary to succinctly explain the nature of the issue

c.Sign the Technician Signature box and click ‘Accept’

7.Click on Save

8.Please add a comment that a NMHA has been created in the ‘Other Issues’ activity for the location selected for the NHMA and set the response for the activity to ‘Y’.
